Systematizing Improvement Interventions of Urgent Care Management in Crohn's Disease
This grant will integrate strategic interventions proven to be effective in reducing urgent care/ER usage into routine CD care, and strives to ensure that high risk CD patients are identified at every IBD Center and gastroenterology practice across the United States, affording them better access to their own providers, allowing them to continue their normal course of care and therapy, while reducing unnecessary patient exposure to reactive steroids and opioids, commonly prescribed by ERs/urgent care centers.
