To expand and evaluate patient-level effectiveness of the Extension for Community Health Outcomes (ECHO) model for underserved people with diabetes on intensive insulin therapy (IIT)—with a focus on type 1 diabetes (T1D)--using a stepped wedge trial design in California and Florida.
