Project Description

CCCare-P tackles an important unmet need in addressing gaps in care for pediatric CD/IBD patients living in Australia and New Zealand. If successful, utilization of the CCCare-P module will have a significant impact on the pediatric CD patient communities of Australia and New Zealand by providing a collaborative environment for access and continuity of up-to-date health-related information on patients' health status, medications, disease progression and plan of care.
