The Diabetes Telehealth Care Beyond High School Study

Project Description

Funding to pilot and assess the health and economic impacts of a new clinical care model for young adults with type 1 diabetes that adapts the in-person shared medical appointment model to a telehealth intervention to allow for remote participation and to use this model to advocate for broader reimbursement of innovative diabetes telehealth interventions.
