Project Description

The objective of this project is to ensure that patients with chronic diseases, and their families, have the right knowledge and skills to access high quality food and steady incomes. The goal is to help communities mitigate challenges related to food security, provide nutritionally balanced diets, and address poverty alleviation measures. This complements the health sector support currently provided by the PEN-plus model of care.
