ECHO Type 1 Diabetes: A Feasibility and Planning Proposal

Project Description

Funding to identify primary care providers (PCPs) who treat people with type 1 diabetes (T1D) and explore how to best provide and implement an ECHO (Extension for Community Healthcare Outcomes) model of care that is designed for the needs of T1D patients who do not receive routine care from endocrinologists, and to establish the feasibility the model for broader implementation.
